چکیده

Background ! Obstructive sleep apnea (OSA) may occur as a result of anatomical or functional obstruction at different levels of the upper and lower airway. Functional obstruction happens mostly during REM (rapid-eye movement) and nonREM 1 (N2) sleep stages. ! PSG (polysomnography) provides the data of sleep O2 saturations, hypopneic and apneic events and abnormal respiratory events per hour, however not indicating the specific etiology of the obstruction. ! Drug-induced sleep endoscopy (DISE) has emerged as a method of 3D evaluation of the upper airway during pharmacologicallyinduced sleep. ! Failure to provide an appropriate depth of anesthesia, imitating the natural sleep patterns during REM and N2 stages may lead to inaccurate evaluation of the functional obstruction.

An updated review of pediatric drug‐induced sleep endoscopy

Objectives Drug-induced sleep endoscopy (DISE) involves assessment of the upper airway using a flexible endoscope while patients are in a pharmacologically-induced sleep-like state. The aim of this article is to review the current literature regarding the role of DISE in children with obstructive sleep apnea (OSA). The indications, typical anesthetic protocol, comparison to other diagnostic mod...

Ideal Drug for Frug Induced Sleep Endoscopy, Protocol Vs Dexmedetomidine- Systematic Review

Drug induced sleep endoscopy is the most important investigation before surgical planning of obstructive sleep apnea patient. It is performed by artificially inducing sleep in a patient and performing nasal endoscopy in order to identify the site of upper airway obstruction. The most appropriate anesthetic agent would be that which induces sleep without any respiratory inhibition. Propofol has ...

Pediatric considerations in sedation for patients with the obstructive sleep apnea syndrome

During sedation and recovery the airway of children with obstructive sleep apnea is vulnerable to collapse. This vulnerability arises from both an inherent collapsibility of the pharyngeal airway in these children and a heightened sensitivity to sedative and anesthetic agents. Pharmacologic and nonpharmacologic support may be required to defend pharyngeal airway patency in children with obstruc...

Anesthesia in multiple sclerosis and obstructive sleep apnea: case report and literature review

While patients with obstructive sleep apnea (OSA) or multiple sclerosis (MS) are at high risk of developing postoperative complications, both of them have special anesthetic considerations in intraoperative and postoperative periods. A careful preoperative evaluation, use of the optimal anesthetic regimen and close postoperative care is essential for these patients. Rarity of coexistence of bot...
